LTVP Medical Examination in Singapore

Not every Long-Term Visit Pass applicant needs a medical examination. ICA states that after in-principle approval you'll be told which tasks are needed, and that these "may include completing a medical check-up". And there are two different LTVPs — one issued by ICA, one by MOM. Establish which is yours before you book anything.

Two different passes share one name

Several clinics describe the LTVP as the pass for the family of an Employment Pass or S Pass holder, and then say ICA requires the medical. Both halves cannot be right. The LTVP for family of work pass holders is issued by MOM; the LTVP for family of Singapore Citizens and Permanent Residents is issued by ICA.

The issuing authority determines the process, the forms and the fees. Work out which route applies to you first.

The basics

What is the LTVP medical examination?

Where ICA requires it, the LTVP medical check-up is part of the completion of formalities stage — after your application has received in-principle approval, and before the pass is issued.

ICA's wording is careful: upon in-principle approval, "you will be informed of the tasks required to complete the formalities. This may include completing a medical check-up." It is not automatic for every applicant.

If you are asked for one, a doctor completes ICA's Medical Examination Report form, which you then upload.

What the examination involves

ICA's Medical Examination Report form asks the examining doctor to certify just two things:

  • Chest X-ray — whether there is any evidence of active tuberculosis. Applicants who are pregnant may be marked exempted due to pregnancy.
  • HIV test — positive, or negative / non-reactive.

What ICA's Medical Examination Report form actually says

Most of what you'll read online about the LTVP medical is a paraphrase of a paraphrase. The rules are printed on ICA's Medical Examination Report form itself:

  • The examination may be done by any registered doctor at a medical clinic licensed to carry out such tests.
  • Renewal applicants must have the examination done in Singapore. Other applicants may have it done in their home country or place of residence.
  • The report will only be accepted if submitted within 3 months of its issuance — so don't have it done too far ahead.
  • HIV testing done in Singapore may be carried out with either rapid or ELISA tests.
  • All fields must be completed. No additional remarks are allowed, and reports that don't meet ICA's requirements will be rejected.

The form also carries a warning: it is an offence under the Immigration Act to make a false statement, representation or declaration.

The sequence

Where the medical fits in ICA's process

For applications submitted on or after 26 June 2024.

  1. In-principle approvalICA informs you of the tasks required to complete the formalities. These may include a medical check-up. The in-principle approval is not an immigration pass and confers no valid stay.
  2. Medical examination, if requiredA doctor completes ICA's Medical Examination Report form.
  3. Upload and verificationComplete all tasks before the in-principle approval expires, or the application is considered withdrawn.
  4. Payment within 14 daysOnce ICA verifies your documents you'll be notified to pay online — S$60 for the LTVP, plus S$30 for a Multiple Journey Visa where applicable.
  5. IssuanceYou'll be told whether you must attend the ICA Services Centre. If not, the LTVP is issued on payment and can be viewed on FileSG the next working day, or via the Singpass app after three working days.

Renewing? ICA advises submitting the renewal application at least three months before your current LTVP expires. Remaining validity is not carried forward.

The LTVP medical examination is not covered by CHAS, Healthier SG or other national subsidy schemes. It is separate from ICA's own fees (S$60 for the LTVP, plus S$30 for a Multiple Journey Visa where applicable). Is a medical examination always required for the LTVP?
No. ICA states that upon in-principle approval you will be informed of the tasks required to complete the formalities, and that these 'may include completing a medical check-up'. Whether you need one depends on your case. Wait to be told before booking.
Who issues the Long-Term Visit Pass — ICA or MOM?
Both, depending on your sponsor. ICA issues the LTVP for family members of Singapore Citizens and Permanent Residents, and for certain other categories. MOM issues an LTVP for certain family members of Employment Pass and S Pass holders who don't qualify for a Dependant's Pass. The authority determines the process, the forms and the fees — so establish which route applies before doing anything.
Who is eligible for an ICA-issued LTVP?
A foreign spouse, unmarried child or parent of a Singapore Citizen or Permanent Resident; the mother or grandmother of a child on a Student's Pass; a graduate from an Institute of Higher Learning seeking employment in Singapore; or a female visitor planning to give birth in Singapore. Foreigners married to Singapore Citizens may be considered for LTVP+.
What tests are in the LTVP medical examination?
ICA's Medical Examination Report form asks the doctor to certify two things: a chest X-ray, indicating whether there is any evidence of active tuberculosis, and an HIV test result. Applicants who are pregnant may be marked as exempted from the chest X-ray. HIV testing done in Singapore may use either a rapid or an ELISA test.
Is the LTVP medical report valid for only three months?
Yes. ICA's Medical Examination Report form states that the report will only be accepted if submitted within 3 months of its issuance — so don't have the examination done too far in advance. Can I skip the medical if I recently submitted one for another pass?
Possibly. ICA's Medical Examination Report form includes a declaration for applicants who have already submitted a medical report containing the same information to ICA or MOM not more than two years ago, when granted another pass. If that applies to you, you may be able to declare it rather than repeat the examination. Confirm with ICA.
Can the LTVP medical examination be done overseas?
ICA's form states that renewal applicants must have the examination done in Singapore, while other applicants may have it done in their home country or place of residence.
How long do I have to complete the formalities?
For applications submitted on or after 26 June 2024, the required tasks must be completed before the in-principle approval expires, or the application is considered withdrawn. Once ICA has verified your documents you will be asked to pay online, and payment must be made within 14 days.
What does the LTVP cost?
ICA's issuance payment may include S$60 for the LTVP and S$30 for a Multiple Journey Visa where applicable. The medical examination is a separate cost, paid to the clinic.
Does the in-principle approval let me stay in Singapore?
No. ICA is explicit: the in-principle approval of an LTVP application is not an immigration pass and does not confer any valid stay on the applicant.
Do I need to visit the ICA Services Centre?
You'll be told after payment. Applicants who are not required to attend in person are issued the LTVP upon payment, and can view it via FileSG the next working day, or through the Singpass app after three working days. Completion of formalities, where required, is strictly by appointment.
When should I renew my LTVP?
ICA advises submitting the renewal application at least three months before your current LTVP expires. The renewed pass runs from the date of issuance — remaining validity on the current pass is not carried forward.
How long does an LTVP application take?
ICA states that most applications are processed within about six weeks, provided all required documents are submitted and in order. An LTVP application by a foreign spouse of a Singapore Citizen can take up to six months.
Do I need to fast for the LTVP medical check-up?
No. Fasting is not required.
What should I bring to the appointment?
Your passport, NRIC or FIN as applicable, and ICA's Medical Examination Report form. If you have been told a medical check-up is required, bring whatever documents ICA has asked you to complete.
What if something abnormal is found?
Your doctor will explain the finding clearly and advise on next steps, which may include further tests or treatment. The outcome may affect your application, and ICA assesses each case individually.
Is the LTVP medical examination subsidised?
No. It is not covered by CHAS, Healthier SG or other national subsidy schemes.
