MOM medical examinationThe Dependant's Pass is issued by MOM, not ICA — and the rules differ. When getting the pass issued, MOM asks for a completed medical examination form or medical declaration form. Read the IPA before you book, then bring the form to us.
First: the Dependant's Pass is a MOM pass, not an ICA one. Rules printed on ICA's Medical Examination Report form — the three-month report validity, the requirement that renewal applicants be examined in Singapore — do not govern a Dependant's Pass.
Second: MOM's own wording at issuance is "completed medical examination form or medical declaration form". Not every dependant needs an examination. Read the IPA letter and its attached declaration form first.

The IPA is a pre-approved single-entry visa. Its validity depends on the main pass holder.

MOM states it plainly: "You should only bring in the foreigner when the pass is approved. They don't need to be in Singapore when you apply and we won't be able to extend their visit pass." If a dependant is already here on a visit pass that expires before the Dependant's Pass is issued, MOM will not extend it.
MOM's Dependant's Pass application form contains a section that applies to foreign-born children aged 12 years and below applying for a new Dependant's Pass or Long-Term Visit Pass.
Before the application can proceed, the child must be issued a “Verification of Vaccination Requirements (for entry to Singapore)” document by the Communicable Diseases Agency (CDA). The CDA reference number is then entered on MOM's form.

The Dependant Pass medical check-up is not covered by CHAS, Healthier SG or other national subsidy schemes. It is separate from MOM's fees — $105 per application and $225 per pass at issuance, plus $30 for a Multiple Journey Visa where applicable.
