Vocational Licence Medical Examination in Singapore

Since 2 February 2026, the vocational licence medical check-up is submitted digitally by your doctor, through LTA and Traffic Police's Harmonised Medical Examination Report (HMER). You no longer upload anything. Bring your notification letter — it carries the QR code we need.

What changed on 2 February 2026

The Harmonised Medical Examination Report (HMER)

Traffic Police and LTA used to issue two separate medical forms. Drivers aged 65 and above sometimes underwent two medical examinations when both agencies' letters arrived in the same year — when only one was ever needed.

From 2 February 2026 the two forms are combined into a single digital report.

  • VL drivers aged 65+ receive one notification letter, from either TP or LTA — not two
  • You undergo one medical assessment
  • No physical form to submit. Your doctor accesses the HMER via a QR code in your letter
  • Your doctor submits it digitally, on your behalf, to both authorities
  • You receive a first SMS confirming submission, and a second SMS within 3 working days confirming it was accepted

Transition period: from 2 February to 31 July 2026, LTA still accepts a completed physical copy of its medical form. If your letter arrived with a paper form attached, bring both. Source: LTA — Harmonised Medical Examination Report (HMER).

How often is a medical examination required?

Different rules apply to your vocational licence (LTA) and your driving licence (Traffic Police).

The statutory upper age limit is 75

LTA states that the statutory upper age limit is 75 years old for Class 4, Class 5 and vocational licence drivers. Drivers who reach 75 on their birthday can no longer hold these licences.

Note that these medical checks keep your VL valid even in years when it isn't due to expire. Missing one puts your licence at risk.

What to bring to your appointment

The single most important item is your letter — the medical letter, renewal notice or ATC letter. Since 2 February 2026 it carries the QR code your doctor scans to open the HMER. Without it, we cannot submit your assessment.

If you have any pre-existing medical conditions, bring a specialist's memo certifying you fit to drive a public service vehicle, along with any existing specialist reports.

Applying for a new vocational licence? Also bring your chest X-ray report, signed by the radiologist.

Referred to a specialist? Come back to the same doctor.

LTA is explicit about this, and it catches people out. If your doctor decides you need a further specialist assessment, obtain the specialist's evaluation and then return to the original doctor, who completes the final assessment and submits the result on your behalf. Starting again with a different clinic means starting the examination again.

Turning 70, 73 or 74?

The on-road assessment

Beyond the medical, VL holders turning 70, 73 and 74 must pass an on-road assessment test as part of renewal.

Where and what to bring

  • Book at Bukit Batok Driving Centre, ComfortDelGro Driving Centre or Singapore Safety Driving Centre
  • Bring your NRIC, renewal notice, and the 2nd SMS from LTA confirming your medical was accepted
  • Not required if you are a non-driver holding only a Bus Attendant's Vocational Licence (BAVL)

Which vehicle?

  • Taxi or private hire car driver: the driving centre provides the car
  • Class 3 bus driver: you must use your own bus
  • Both licences? You take the assessment twice — once with a car, once with a bus
  • Modified vehicle (spinner knob, left-foot accelerator): take the assessment at Tan Tock Seng Hospital's Occupational Therapy Clinic

Note the sequence: your medical must be accepted (the 2nd SMS) before you can book the on-road assessment. Leave time for both.

One visit, and we submit the HMER for you.

  2. Tell us your licence classThe doctor applies the medical standard for that class. Class 4/5 and all vocational licences are Group 2, with higher standards than Class 2/3.
  3. Medical examinationPlus a chest X-ray if you are applying for a new vocational licence. Bring a specialist's memo if you have pre-existing conditions.
  4. We submit it digitallyYour doctor submits the HMER to LTA and Traffic Police on your behalf. Nothing for you to upload.
  5. Two SMS messagesThe first confirms submission. The second, within 3 working days, confirms it was accepted — keep it, you may need to show it.

Chest X-ray can be completed on-site at our Suntec, Macpherson and Toa Payoh clinics. LTA advises calling any clinic beforehand to confirm the 'Fitness to Drive' service is available — please do call us.

Vocational licence medical check-up cost

LTA states that the cost of the medical examination is determined by individual clinics, and is separate from LTA's own fees ($40 for a VL application, $40 for a VL renewal). It is not covered by CHAS, Healthier SG or other national subsidy schemes. Contact us for current pricing, and tell us whether a chest X-ray is required.

Vocational licence medical examination FAQs

How often do I need a vocational licence medical examination?
It depends on your age. LTA requires VL holders aged 50 to 64 to undergo a medical check-up every 2 years, and VL holders aged 65 and above every year until the age of 74. These checks keep your VL valid even in years when it isn't due to expire.

What is the HMER?
The Harmonised Medical Examination Report. From 2 February 2026, Traffic Police and LTA combined their two medical forms into one digital form. Your doctor accesses it through a QR code in your notification letter, and submits the result directly to the authorities on your behalf. You no longer scan or upload anything.
How will I know my report has been submitted?
By SMS. You receive a first SMS confirming your doctor has submitted the HMER, and a second SMS within 3 working days confirming the submission has been accepted.
Do I still need to bring a paper form?
During the transition period from 2 February 2026 to 31 July 2026, LTA still accepts a completed physical copy of its medical form. If your notification letter arrived with a paper form attached, bring both the letter and the form to your appointment.
Do I need a chest X-ray?
For a new vocational licence application, yes — LTA requires a medical check-up and a chest X-ray examination after your VL application is approved, and the chest X-ray report must be signed by a radiologist. For routine renewals and periodic medical check-ups, a chest X-ray is not part of the standard requirement.
What should I bring to the appointment?
Your notification letter, renewal notice or ATC letter — it contains the QR code your doctor needs to access the HMER. If you have any pre-existing medical conditions, bring a specialist's memo certifying you fit to drive a public service vehicle. For a new VL application, also bring the chest X-ray report signed by the radiologist.
Which clinics can do the vocational licence medical check-up?
LTA states the examination may be conducted by any Singapore-registered medical practitioner — a doctor registered to practise medicine in Singapore under the Medical Registration Act 1997. However, not every clinic offers 'Fitness to Drive' examinations, so LTA advises calling the clinic beforehand to confirm the service is available.
What if I'm referred to a specialist?
You must return to the same doctor who started your examination. LTA is explicit: if you are referred for further specialist assessment, obtain the specialist's evaluation and then go back to your original medical practitioner, who completes the final assessment and submits the result on your behalf.
What is the on-road assessment, and who needs it?
VL holders turning 70, 73 and 74 must take an additional on-road assessment test as part of renewal. It is not required if you are a non-driver holding only a Bus Attendant's Vocational Licence (BAVL). Book at Bukit Batok Driving Centre, ComfortDelGro Driving Centre or Singapore Safety Driving Centre, bringing your NRIC, renewal notice and the second SMS from LTA.
Do I use my own vehicle for the on-road assessment?
If you are a taxi or private hire car driver, the driving centre provides the car. If you are a Class 3 bus driver, you must use your own bus. If you hold vocational licences for both, you take the assessment twice — once with a car and once with a bus. If you drive a modified vehicle, such as one with a spinner knob or left-foot accelerator, take the assessment at Tan Tock Seng Hospital's Occupational Therapy Clinic.
When will I be notified that my medical is due?
For periodic medical requirements, LTA or Traffic Police sends a medical letter to your registered address 2 to 3 months before the check-up is due. For renewals, the renewal notice arrives 1 to 2 months before your licence expires. For new applications, the Approval-to-Attend-Course letter is emailed within 15 working days of your application being approved.
What happens if I fail the medical examination?
LTA states plainly that you will not be able to hold a vocational licence if you fail the medical examination for your VL — and you will not be permitted to drive if you fail it for your driving licence. Having a medical condition does not automatically disqualify you; a specialist's assessment may be needed.
Why do vocational drivers have higher medical standards?
Under the HMER, holders of Class 2 and Class 3 driving licences are Group 1. Holders of Class 4 and Class 5 driving licences, and all vocational licences, are Group 2 — and higher medical standards apply to Group 2.
